Welcome to the Disclosure Request Form page.  This is intended to make it easy for Realtors, Closing Agents and/or Homeowners to request a Community Disclosure Package when selling a property within our community.  The cost to prepare the disclosure package is $100.00 and shall be paid at the time of the order.  The payment can be sent by Venmo, check to any current HOA Board member or to the PO Box of our community.  Should you have any questions, please contact the Director at Large for the community at the email below.
